'Pre-Registration' With The Practice

The forms

Please complete the below linked paperwork to register with the practice. There are 3 forms:

GMS1 form (purple) - this is the official form for you to request registration here.
Registration form (adult and child versions) - this is for some important additional information about you.
Sharing your information form – this is about electronic sharing of aspects of your medical records.

If there are any problems or queries with your registration forms then you will be contacted by our admin team. If you do not hear anything after two weeks, then you can assume that you are registered with the Practice. If you take prescription medication, please book an appointment with a GP to have this set up and reviewed. If you are generally well, then you do not need to see a GP, but are still welcome to come for a new patient health check with
our healthcare assistants.

Our online registration form are available below, please print off and either scan and send the documents to or you can take the forms and hand these in to the reception staff, or you can post the forms tot he surgery. 

You will need to hand in a completed GMS 1 form and a practice registration form to register at the practice this is required for each person registering.  If you wish to opt out of the Summary Care Record or the Oxfordshire Case Summary then you will need to print off one of the forms and submit this as well as your registration forms
